This study aims to describe social network and social participation and to assess associations with depressive symptoms in older persons with increased risk for dementia in Germany. We conducted a cross-sectional observational study in primary care patients (aged 60-77) as part of a multicenter cluster-randomized controlled trial (AgeWell.de). We present descriptive and multivariate analyses for social networks (Lubben Social Network Scale and subscales) and social participation (item list of social activities) and analyze associations of these variables with depressive symptoms (Geriatric (...) Depression Scale). Of 1030 included patients, 17.2% were at risk for social isolation (Lubben Social Network Scale < 12). Looking at the subscales, a reduced non-family network was found almost twice as often as a reduced family network. Patients with depressive symptoms had significantly smaller social networks than patients without depression (p < 0.001). They rather engaged in social activities of low involvement level or no weekly social activity at all (p < 0.001). The study shows associations of depressive symptoms with a decreased social network and less social participation in elderly participants. Sufficient non-family contacts and weekly social activities seem to play an important role in mental health and should be encouraged in elderly primary care patients. (shrink)

"Kant, Kantianism and Idealism" presents an overview of German Idealism, the major movement in philosophy from the late 18th to the middle of the 19th Century. The period was dominated by Kant, Fichte, Schelling and Hegel, whose work influenced not just philosophy, but also art, theology and politics. The volume covers not only these major figures but also their main followers and interpreters. These include Kant's younger contemporary Herder, his early critics such as Jacobi, Reinhold, and Maimon, and his readers (...) Schiller and Schlegel - who shaped much of the subsequent reception of Kant in art, literature and aesthetics - as well as Schopenhauer, whose unique appropriation and criticism of theories of cognition later had a decisive influence on Nietzsche. The "Young Hegelians" - such as Bruno Bauer, Ludwig Feuerbach, and David Friedrich Strauss, whose writings would influence Engels and Marx - are also discussed. The influence of Kant and German Idealism also extended into France, shaping the thought of such figures as Saint-Simon, Fourier, and Proudhon, whose work would prove decisive for subsequent philosophical, political, and economic thinking in Europe in the second half of the 19th century. A psychiatric advance directive (PAD) is a communication tool that promotes patients’ autonomy and gives capacitated adults who live with serious mental illnesses the ability to record their preferences for care and designate a proxy decision maker before a healthcare crisis. Despite a high degree of interest by patients and previous studies that recommend that clinicians facilitate the completion of PADs, the rate of implementation of PADs remains low. Research indicates that many clinicians lack the necessary experience to facilitate the (...) completion of PADs and to use them, and, as a consequence, do not effectively engage patients about PADs. This study developed practical recommendations for clinicians to improve their ability to communicate and facilitate PADs. We (1) thematically analyze definitions of PADs published in 118 articles across disciplines, and (2) present our recommendations for enhanced communication in clinical practice that emphasize patient-centeredness, usefulness, and clarity, aligned with evidence-based practices that put patients’ autonomy and understanding first. While there is no one-sizefits- all script to engage patients in complex conversations, our recommended strategies include an emphasis on patients’ autonomy, the adaptation of word choices, the use of metaphor not simile, and checking for patients’ understanding as effective methods of clinical communication. Increasing emphasis on patient self-management, including having patients advocate for their needs and priorities, is generally a good thing, but it is not always wanted or attainable by patients. The aim of this critical ethical review is to deepen the current discourse in patient self-advocacy by exposing various situations in which patients struggle to self-advocate. Using examples from oncology patient populations, we disambiguate different notions of self-advocacy and then present limits to the more demanding varieties (i.e., health-related, trust-based, and psychological); (...) we argue that these limits create ethical dilemmas with respect to whether it is always desirable to encourage patients to self-advocate. We conclude that self-advocacy can be both under and overrated with respect to how much it benefits the patient with cancer, with many instances being indeterminate. Ultimately, providers must understand the patient's perspective relative to the challenges they are experiencing and work with them to meet their needs. (shrink)

Several philosophers of science and metaphysicians claim that the dispositional properties of fundamental particles, such as the mass, charge, and spin of electrons, are ungrounded in any further properties. It is assumed by those making this argument that such properties are intrinsic, and thus if they are grounded at all they must be grounded intrinsically. However, this paper advances an argument, with one empirical premise and one metaphysical premise, for the claim that mass is extrinsically grounded and is thus an (...) extrinsic disposition. Although the argument concerns mass characterized as a disposition, it applies equally well whether mass is a categorical or dispositional property; however, the dispositional nature of mass is relevant to some important objections and implications discussed. This paper summarizes (in German) recent tendencies in the philosophy of the life sciences.

In the past decade, digital technology, fiber optics, cellular phones, satellite television, home computers, and the Internet have substantially transformed business, education, and leisure practices. These technologies are becoming so integrated into our daily routines that their ubiquity often goes unnoticed. We are, nonetheless, in the midst of a telecommunications revolution, and the healthcare industry is becoming a major player. The burgeoning field of home-based telemedicine is evidence of this.

What ethical principles should autonomous machines follow? How do we implement these principles, and how do we evaluate these implementations? These are some of the critical questions Vivek Nallur asks in his essay “Landscape of Machine Implemented Ethics (2020).” He provides a broad, insightful survey of answers to these questions, especially focused on the implementation question. In this commentary, I will first critically summarize the main themes and conclusions of Nallur’s essay and then expand upon the landscape that Nallur presents (...) by suggesting additional approaches to machine ethics. The approaches I discuss reflect normative ethical theories and need not be applied only to machines, although machine ethics is the focus here. The overall goal is to open up further questions and research possibilities as society searches for the best approach to machine ethics. Given that artificial moral agents—such as autonomous vehicles, lethal autonomous weapons, and automated financial trading systems—are now part of the socio-ethical equation, we should morally evaluate their behavior. How should artificial moral agents make decisions? Is one moral theory better suited than others for machine ethics? After briefly overviewing the dominant ethical approaches for building morality into machines, this paper discusses a recent proposal, put forward by Don Howard and Ioan Muntean (2016, 2017), for an artificial moral agent based on (...) virtue theory. While the virtuous artificial moral agent has various strengths, this paper argues that a rule-based utilitarian approach (in contrast to a strict act-utilitarian approach) is superior because it can capture the most important features of the virtue-theoretic approach while realizing additional significant benefits. Specifically, a 2-level utilitarian artificial moral agent incorporating both established moral rules and a utility calculator is especially well-suited for machine ethics. (shrink)

Philosophers debate the death of philosophy as much as they debate the death of God. Kant claimed responsibility for both philosophy's beginning and end, while Heidegger argued it concluded with Nietzsche. In the twentieth century, figures as diverse as John Austin and Richard Rorty have proclaimed philosophy's end, with some even calling for the advent of "postphilosophy." In an effort to make sense of these conflicting positions—which often say as much about the philosopher as his subject—Isabelle Thomas-Fogiel undertakes the (...) first systematic treatment of "the end of philosophy," while also recasting the history of western thought itself. Thomas-Fogiel begins with postphilosophical claims such as scientism, which she reveals to be self-refuting, for they subsume philosophy into the branches of the natural sciences. She discovers similar issues in Rorty's skepticism and strands of continental thought. Revisiting the work of late-nineteenth and early-twentieth-century philosophers, when the split between analytical and continental philosophy began, Thomas-Fogiel finds both traditions followed the same path—the road of reference—which ultimately led to self-contradiction. This phenomenon, whether valorized or condemned, has been understood as the death of philosophy. Tracing this pattern from Quine to Rorty, from Heidegger to Levinas and Habermas, Thomas-Fogiel reveals the self-contradiction at the core of their claims while also carving an alternative path through self-reference. Trained under the French philosopher Bernard Bourgeois, she remakes philosophy in exciting new ways for the twenty-first century. (shrink)

The past decade has seen a rapid development and proliferation of sophisticated computer systems in organizations. Designers, however, have minimized the importance of security control systems, (except for those systems where data security and access control have obviously been of major importance). The result is an increasing recognition that computer systems security is often easily compromised.This research will provide the initial step in assessing ways in which attorneys retained to prosecute computer crimes and computer people who discover these violations can (...) work together to strengthen both our computer systems to thwart violators and the laws that are currently on the books that can be used to prosecute violators. What is the ethical impact of artificial intelligence assistants on human lives, and specifically how much do they threaten our individual autonomy? Recently, as part of forming an ethical framework for thinking about the impact of AI assistants on our lives, John Danaher claims that if the external automaticity generated by the use of AI assistants threatens our autonomy and is therefore ethically problematic, then the internal automaticity we already live with should be viewed in the same way. He takes (...) advantage of this paradox of internal automaticity to downplay the threats of external automaticity to our autonomy. We respond in this paper by challenging the legitimacy of the paradox. While Danaher assumes that internal and external automaticity are roughly equivalent, we argue that there are reasons why we should accept a large degree of internal automaticity, that it is actually essential to our sense of autonomy, and as such it is ethically good; however, the same does not go for external automaticity. Therefore, the similarity between the two is not as powerful as the paradox presumes. In conclusion, we make practical recommendations for how to better manage the integration of AI assistants into society. (shrink)

If the universe and God are identical, as pantheism holds, how can we reconcile the supposed unity of God with the apparent dis-unity of the universe’s elements? I argue that a powers ontology, which generates a form of pantheism under plausible assumptions, is apt to solve the problem of unity. There is reason to think that the directedness of powers is equivalent to the directedness, or intentionality, of mental states. This implies that intentionality is a feature of the physical world (...) at large, not just the mind. As Pfeifer argues, this physical intentionality represents a mild form of panpsychism that is interpretable as a brand of pantheism. I argue that the theory of powers holism explains the unity of the universe’s elements and thus the unity of a pantheistic God, and that powers holism is strengthened by fully recognizing the role of physical intentionality. Dispositional essentialism maintains that all sparse properties are essentially powerful. Two conceptions of sparse properties appear compatible with dispositional essentialism: sparse properties as pure powers or as powerful qualities. This paper compares the two views, criticizes the powerful qualities view, and then develops a new theory of pure powers, termed Point Theory. This theory neutralizes the main advantage powerful qualities appear to possess over pure powers—explaining the existence of powers during latency periods. The paper discusses the relation between powers and (...) space-time points, whether pure powers need to occupy space, and how to account for the movement of pure powers through space-time. Given Point Theory, dispositional essentialists should maintain that sparse properties are pure powers. (shrink)

Experimenters claim some nonhuman mammals have metacognition. If correct, the results indicate some animal minds are more complex than ordinarily presumed. However, some philosophers argue for a deflationary reading of metacognition experiments, suggesting that the results can be explained in first-order terms. We agree with the deflationary interpretation of the data but we argue that the metacognition research forces the need to recognize a heretofore underappreciated feature in the theory of animal minds, which we call Unity. The disparate mental states (...) of an animal must be unified if deflationary accounts of metacognition are to hold and untoward implications avoided. Furthermore, once Unity is acknowledged, the deflationary interpretation of the experiments reveals an elevated moral standing for the nonhumans in question. (shrink)

After decades of specialization within the sciences, the development and application of implantable microchips and biosensors are now being made possible by a growing convergence among seemingly disparate scientific disciplines including, among others, biology, informatics, chemistry, and engineering. This convergence of diverse scientific disciplines is the basis for the creation of new technologies that will have significant medical potential. As of today, implantable microchips and biosensors are being used as mental prostheses to compensate for a loss of normal function, to (...) remotely monitor patients' vital signs, to control the delivery of medications, and to communicate with geographically distant healthcare professionals and the outside environment.
